HM 008 Error at initialization

I have been getting the HM 0008 error upon initializing the machine this morning. The proximity switches are all working and the initializing stops when checking the Y axis. The machine goes back and triggers both of the proximity switches and kicks forward about 1/4 inch and stops - does not bump back a few times; then error code shows up.
There was a screw hanging loose upon inspection

I have adjusted the screw a few times and adjusted the other side as well. No joy.
I do have a request for help in to Carbide support and have been very satisfied in the past with their help, but I am impatient and would like to fix it sooner rather than later. I am officially obsessed with the issue.

and that is fail to pull off.

Do you have a BitZero? Where is the groundlead for it? Are they touching?

What colour is your BitSetter showing?

If you slowly/gently move the machine to the center of motion for each axis, then power up and connect and go to Settings | Debug are any switches stuck on?

I do not have a bit zero. The bit setter is showing red until I push it and then green. When I move the machine manually while powered down and turn it back on, the sensors all show off when I view the debug.
When I initialize the machine, it seems it is unable to pull off of the X axis. I have changed out the switch with no change in results.
