Shapeoko 4 XL getting lost in middle of project

So I have had my shapeoko for a year now and have done quite a few projects. My latest project is a Tooth plaque for a dentist retiring from the Navy. I have a gray scale image of the plaque and had started cutting ( run time 2.5hrs for first rough cut pass). This is the second time running this as the first time it decided to take a sharp turn into the middle of the piece after 1hr…this time it got lost in mid-cut and drove the bit 1/2" into the piece and started to cut and pull. I have attached pictures. any help would be great. It seems like a lot more of these issues have started once I used the new carbide create with the no gcode. I will try and upload the file as well.

How far back from the front of the machine are you placing your stock?
The only possible cause I could see for this is if the machine loses steps at the back of the table. I’ve had projects do that to me where they were just below the limits of my machine, but I set zero too far back and it thought it was cutting correctly, but it lost steps when it hit the rear limits.

It is pretty far back, however, I moved the center point so it would clear it as that was the issue the first go around. this had already cleared material and was 80% done when it lost its place. it was doing a rapid movement to the new location when it stuttered and then drove straight down into the material. I had cleared away the sawdust from the tracks and everything but I guess there could have been some small piece that stopped a wheel. not sure…might take it apart and make sure all the wheels are moving well.
