I’ve wasted so mush wood trying this over and over and I have no idea what’s going wrong! These are just simple letters. I’m using Shapeoko Pro XXL with carbide motion on the default settings. The board is strapped down tight. What could be going wrong here? SEE PICS
Have you seen:

Is your machine mechanically sound with all wiring and connectors in good condition and secure?
What does the 3D preview look like in Carbide Create?
Where is the origin set in the file?

How are you setting zero relative to your stock?

Upload your .c2d file and a photo showing the stock/part still in place on the machine with the machine at the origin?
Looks like you lost some steps in X before the Vcarve corner cleanup; did you bump into something on the left side? possibly a dust collection or a cable?
Post your file? This has happened multiple times in the same spots?
After staring at this a bit, maybe set screws holding the pulley on the x axis stepper motor need tightened up. (First be sure the set screw is aligned with the flat on the stepper motor shaft).
recommend to re-check the original CAD-file (dxf?). Looks like there are some dots somewhere that are hidden to the eye, can be found with “select all” and delete them.
I thought the same as you but a few things changed my mind:
- I think the only tool path that would do this would be an outside contour and this picture looks like an advanced V carve (maybe). We can’t really know the tool path until the file is posted.
- The out of place holes are all offset to the right about the same distance from a letter area that would require v carve cleanup. So I think David Hood is correct in that respect.
Problems with the last bullet point are:
Corresponding Vcarve corner paths are not seen at the bottoms of the letters, shouldn’t there be? Or is this an odd font? Maybe the carve was stopped before it completed? Only the middle arm of the “E” needed cleanup?
Look closely and there are irregularities on the left sides of the bottoms of the letters. This may imply there is play (right AND left ) in X movement.
We really need an original file to confirm what the carving should be.
Cheers
