I’ve done it both ways. I don’t think the outlining is necessary. Probably depends on the design.

I engraved with the full and diamond tip just like you, but I also outlined, on about half or so the outline is off
Sounds mechanical. Like you lost steps. Check your belts, eccentrics, etc.
