Mesh editing - Localized push/pulls

I have been using the imagetorelief app with good success, except for black areas ( like dog eyes/nose or tires on a car). The app sends those areas to the back.

I have tried multiple setting changes with limited success. I have flipped the grey scale of the image at times, but I would like to be able to select areas then pull them up.

I have been using meshmixer, but hardly an expect.

What apps would you folks suggest ?

I do have Blender, but I have not jumped in as yet.

Blender or Zbrush. I use Zbrush. I can bump up the z in masked area from behind without ruining the surface textures.

Time for a redo

The imagerorelief results in very large meshes (+12 million) depending on image resolution.
I probably should reduce this.

I could decimate the mesh or reduce the image size.
Do you have pros and cons for either process ?

I probably should aim for a vertice spacing of .005" on the final carve space. Again suggestions ?

I use the pull brush for that. B-M-V is the shortcut. And of course the smoothing brush which is the same brush but with the shift key held down.
