Pro x axis motor pulley location

Been having longtime issues with x axis shifting during cut.
Removed motor today and found set screws loose and not on flat (see witness marks) if I moved pulley flush as in y axis the belt was out of line.
Also both y axis screws are tight but not in contact with flat. Am I expected to grind more flat on x motor. Are y axis flats supposed to utilized?
Is x pulley supposed to be flipped?
pulleys supplied this way from manufacturer.

The pulley should be positioned so that the belt runs straight and doesn’t unduly rub.

Are you cutting slots just as narrow as the tool?

Where possible avoid slotting and add geometry and cut as a pocket

and/or

and consider leaving a roughing clearance and taking a finishing pass.
